boa tarde pessoal, preciso de uma ajuda de vocês com o forms abaixo

preciso que ele seja enviado por email para o cliente mas não estou conseguindo... algum pra dar um help?

Tu postou apenas o formulário HTML.

No action tu precisa indicar um arquivo em php que recebe os dados do formulário e envie via e-mail.

Recomendo utilizar a classe php mailer.
